I really hate using 'self-appraisal forms' to evalute one's performance.
Below is an appraisal form which I  filled recently but didn't submit ;)

1)What do you consider to be your most important achievements of the past year/since DOJ?
Getting another offer and the relieving letter and last working day from you guys.

2)What could you have done better
Should have examined about this company before accepting the offer.

3)sense of urgency:
Yes,Urgently want  to get out of here.

4)technical capability:
Useless here

5)positive energy / teamwork:
Not applicable 

6)quality of execution/ attention to detail:
quality? what's that? 

7)Input/ Suggestions:
blah blah blah. (anyway they are useless)
